Our Coverage

5-Year Warranty — Crowns, Bridges, Fillings, and Dental Implant Restorations
Years 1–3: Full replacement at no cost
Years 4–5: Lab fee only applies

Invisalign® Treatment: 3 years of coverage from treatment start date
Surgical Dental Implants (Placement): 1-year coverage from placement date

Exclusions

This warranty does not apply to:
– Periodontal (gum) treatment
– Endodontic (root canal) treatment
– Orthodontic relapse due to non-compliance with retainers
– Whitening procedures
– Trauma-related damage
– Complete or partial dentures

Warranty Requirements

To remain eligible for coverage:

• Maintain regular periodontal maintenance and recall appointments (every 3, 4, or 6 months as prescribed)
• Follow home care and maintenance instructions
• Use night guards, retainers, or other prescribed protective devices
• Avoid neglect, trauma, or outside alterations

Failures due to poor home care, recurrent decay, or missed maintenance visits are not covered.

Your Benefits

If a covered restoration fails within the warranty period and the failure is not due to neglect or trauma, we will redo or replace the restoration at no charge.

– Years 1–3: Full replacement at no cost
– Years 4–5: Lab fee only applies
– No cash refunds are available
– Warranty becomes void if you:

• Change to another dentist
• Miss hygiene or recall appointments
• Decline recommended maintenance or treatment
